Root planing is the primary step in non-surgical therapy.  The goal of root planing is to remove the porous, mineralized deposits of dead bacteria (which contain live bacteria also) from the roots.  Once the bacteria-containing calculus is removed, the patient will be able to clean the smooth surface better and the gums tend to shrink.   If there is adequate shrinkage, some gum pockets might potentially be reduced to a more normal state such that the patient will be able to access the bacteria in them with flossing and brushing.   That is the reason root planing is usually accomplished first, to see if both the calculus and the pockets can be resolved without surgery (see "What is Periodontitis" tab).   However, if the pockets are not eliminated, surgery will then be required.
